A pioneering pilot human histology study investigated this very process after a single 20-minute RF treatment. When they analyzed the tissue, the results were striking.
The levels of caspase-7, a key executioner protein in the apoptosis pathway, increased by a staggering 511% just 24 hours post-treatment.

An MRI study on a technology combining RF with muscle stimulation (HIFES) followed 33 subjects. The scans don’t lie. The overall submental volume decreased by 25.12% at 1 month and by an incredible 36.20% at the 3-month mark. The submental fat itself was reduced by 20.54% at 1 month and 30.37% at 3 months

RF + Deoxycholic Acid: A recent controlled study from 2025 compared using subdermal RF lipolysis alone against RF plus phosphatidylcholine/deoxycholic acid (PC+DCA) injections, which are the active ingredients in injectable fat dissolvers.
The RF-only group saw a respectable 20.3% mean reduction in submental thickness. However, the combination group achieved a 31.6% reduction.
The statistical analysis confirmed a significantly greater improvement with the combined therapy, with the study concluding that “adding PC+DCA injections confers statistically and clinically superior outcomes”.
The RF creates a controlled injury and tissue remodeling effect, making the area potentially more receptive to the fat-dissolving action of the injectable, all while tightening the skin over the now-reduced fat pad.
Beyond the Fat Cell: A Three-Layer Approach
The appearance of a double chin is rarely just about fat. It’s a complex interplay of three distinct layers. This is why a holistic approach like the one advanced RF systems provide is so effective.
Targeting All Contributing Layers: Newer technologies simultaneously deliver RF energy and High-Intensity Focused Electrical Stimulation (HIFES) to address the problem on three fronts:
